We are seeking a Case Manager in our Family Visitation Program to join our team at our program in Wilmington, DE.

Position Details

  • Coordinates resources and information pertinent to client needs.
  • Efficiently documents all coordination of resources.
  • Coordinates referrals for client specific discharge planning; ensures that the family actively participates in creating a multi-faceted discharge plan; one which represents the family's choice for service providers, and community resources.

  • Monitors record keeping, compliance and authorized units remaining, ensures that client charts are up-to-date and include all necessary documentation, including documentation of significant incidents within 24 hours as needed (incident reports).
  • Monitors/assesses and documents safety within resource homes.
  • Completes necessary documentation of each case, which complies with the standards set by Merakey, county MHDS/CASSP, OMHSAS, and other licensing/regulatory/payor organizations.

Earn: $43,000+ per yr.
