Horizon House is a nationally recognized provider of services to adults with support needs due to developmental disabilities, behavioral health issues and substance abuse. Horizon House is Pennsylvania's oldest psychiatric rehabilitation agency. Since 1952, Horizon House has been dedicated to helping people with mental illness, drug and alcohol addictions, developmental/intellectual/disabilities and homelessness to help stabilize their lives and to help them live as independently as possible in the community. With over 100 locations throughout Southeastern Pennsylvania and the state of Delaware, Horizon House provides services to a diverse population focusing on individual strength and choice. We are seeking qualified candidates interested in joining our Assertive Community Treatment (ACT) team at our Wilmington, Delaware location. The Licensed Practical Nurse (LPN) as part of the ACT multidisciplinary team provides the coordination of both the behavioral and physical health care needs of the program participants, monitors participants medication and provides psychiatric nursing services consistent with physician's orders. Some of the responsibilities include the following. -
Assist in the development, implementation, evaluation and revision of recovery plans in collaboration with the participants and assigned staff. -
Maintains a case load of assigned participants and completes assigned paperwork in a timely manner. -
Act as a liaison consulting with community agencies and families to maintain coordination of care in the treatment process. -
Provides ongoing assessment of participant's mental health symptoms and responds to recovery plans. -
Educates and supervises participants in self-administration of medication. -
Ensures annual review of the participants physical condition and assist access to and encourage compliance with treatment for identified physical problems.
