Job Description/Essential Functions:

Horizon House is a nationally recognized provider of services to adults with support needs due to developmental disabilities, behavioral health issues and substance abuse. Horizon House is Pennsylvania's oldest psychiatric rehabilitation agency. Since 1952, Horizon House has been dedicated to helping people with mental illness, drug and alcohol addictions, developmental/intellectual disabilities and homelessness to help stabilize their lives and to help them live as independently as possible in the community. With over 100 locations throughout Southeastern Pennsylvania and the state of Delaware Horizon House provides services to a diverse population focusing on individual strength and choice.

We are seeking a qualified candidate interested in joining our Assertive Community Treatment (ACT) in Wilmington, Delaware.

The Clinician Registered Nurse as part of the ACT multidisciplinary team provides the coordination of both the behavioral and physical health care needs of the program participants, monitors participants medication and provides psychiatric nursing services consistent with physician's orders. Some of the responsibilities include the following:

  • Assist in the development, implementation, evaluation and revision of recovery plans in collaboration with the participants and assigned staff.

  • Maintains a case load of ACT participants and completes assigned paperwork in a timely manner.

  • Act as a liaison consulting with community agencies and families to maintain coordination of care in the treatment process.

  • Provides ongoing assessment of participant's mental health symptoms and responds to recovery plans ensuring appropriate and early intervention.

  • Educates and supervises participants in self administration of medication.

  • Ensures annual review of the participant's physical condition, and assist access to and encourage compliance with treatment for identified physical problems.

Job Qualifications:
  • A Registered Nurse with a current and valid nursing license in the state of Delaware.

  • Bachelor's degree in nursing preferred.

  • Knowledge of mental health and substance abuse systems and supports.

  • Good communication skills both oral and written.

  • Ability to work independently and as part of a focused team.

  • Strong organizational and analytical skills.

  • Possess a valid driver's license with an acceptable driving record.

  • Skills and competence to establish supportive trusting relationships with persons with severe and persistent mental illness and respect for participants' rights and personal preference in treatment are essential.

Scope:

  • Frequent talking and listening.

  • Requires the ability to lift, carry, hold, kneel, reach, stack, stoop, and bend, which may include direct intervention with participants.

  • Local travel required, including transporting program participants.

  • May be required to report to work during emergencies, including inclement weather.

  • May require crisis intervention in emergency situations.

  • Scheduling flexibility and possible on-call work are necessary.
