Program Overview

Community Solutions, Inc. is seeking a dedicated candidate to fill the role of a Fulltime Case Manager Supervisor, for our FBOP-RRC program located in Wilmington, DE. Our Case Management team serve clients coming out of Federal prison and transitioning back into the community. The Case Manger Supervisor is an integral part of the whole residential program overseeing the team of Case Managers who work with our clients to develop transition plans, provide group counseling and work toward client discharge and aftercare services. The Case Manager supervisor will oversee staff in all areas of staff development, performance and policy, case management, and reporting data by thoughtfully assigning workload to challenge and grow individual skills. As the Case Manager Supervisor you will also interpret and review the Case Management data, create insightful and comprehensible reports and lead the facilitation of various groups or meetings. These are programs for state and federal work release clients who live in a congregate setting and are supported as they obtain full-time employment and meet financial and self-sufficiency goals. The programs administer assessments, monitor compliance with treatment plan goals and involve clients in direct treatment services that address their criminogenic tendencies. Random breathalyzer or urinalysis, cognitive behavioral groups, home confinement monitoring, and case management to assist clients in maintaining stability leading to successful discharge.

Position Overview

  • Perform all duties and responsibilities of case manager, including carrying a caseload as required by program needs, as relates to counseling, case record management, intake assessment, discharge preparation, aftercare services, security, resource management and facility maintenance.
  • Supervise and oversee Case Manager Team, relating to security, shift emergencies and other incidents.
  • Review all client related paperwork, regarding content, timeliness and quality. Review all client files on a monthly basis for organization and content.
  • Monitor client compliance with treatment stipulations and recommend modifications to program director.
  • Assist program director in developing, leading and supervising client group meetings, program services and counseling sessions. Monitor and evaluate group activities on an ongoing basis.
  • BOP case management sessions are to be held at least once per week (every 7 calendar days) for their first 6 weeks.
  • BOP Clients: Submit weekly progress reports for the first (6) six weeks and bi-weekly progress reports thereafter.
  • Complete and submit discharge plan for BOP client 60 days before scheduled release date.
  • Prepare, plan and facilitate Transitional Skills and R&R group-sessions
  • Record all sessions with clients and follow procedure to upload these recordings for quality assurance review.
  • Actively participate in quality assurance review session and apply new strategies and techniques presented.
  • Actively participate in PRT meetings by presenting new clients assessments and ISP, updates on clients nearing discharge, provide and elicit peer support for client issues.
  • Maintain client files and ensure compliance with Agency policy regarding confidential information.
  • Complete incident reports as needed and submit to PD prior to leaving shift.
  • Register for and attend all required trainings and boosters including but not limited to MI, Case Management and Assessments.
  • Documentation of group and individual sessions are to be entered into the agency data systems on the same day the information is received as well as funder database.
  • Must maintain a minimum of 75% completion rate for each intervention facilitated.
  • Maintain a working knowledge of and abide by the Agency's programs, policies, procedures and code of ethics. Perform other tasks as assigned by supervisor.


Schedule: Mon/Tue/Fri 9am-5pm & Wed/Thur. 1pm-9pm

Annual Salary: $ 44166.67

Qualifications:

  • Bachelor's Degree in Social Science field and one year experience in case management or similar field. *Bachelors required for FBOP programs*
  • Pass a Federal Bureau of Prisons background check.
  • Attend 40hrs of FBOP required training, prior to working FBOP clients.
  • Education may be substituted with relevant experience on a one year for one year basis.
  • Excellent oral, written and interpersonal skills desired.
  • Ability to engage and develop collaborations with community agencies.
  • Previous assessment experience preferred
  • Experience with Microsoft Office
