Job Description/Essential Functions:

Horizon House is a nationally recognized provider of services to adults with support needs due to developmental disabilities, behavioral health issues and substance abuse. Horizon House is Pennsylvania's oldest psychiatric rehabilitation agency. Since 1952, Horizon House has been dedicated to helping people with mental illness, drug and alcohol addictions,developmental/intellectual disabilities and homelessness to help stabilize their lives and to help them live as independently as possible in the community. With over 100 locations throughout Southeastern Pennsylvania and the state of Delaware, Horizon House provides services to a diverse population focusing on individual strength and choice.

We are seeking a qualified candidate interested in joining our Residential Service team at our locations in Wilmington, Delaware.

The Clinician Registered Nurse as apart of the Residential Service team provides the coordination of the behavioral and physical health care needs of the program participants, monitors participant's medication, and provides psychiatric nursing services consistent with physician's orders. Some of the responsibilities would include the following:

  • Assist in the development, implementation, evaluation and revision of recovery plans in collaboration with the participant and assigned staff.

  • Act as a liaison and consult with community agencies and families to maintain coordination of care in the treatment process.

  • Provide ongoing assessment of participant's mental health symptoms and responses to recovery plans ensuring appropriate and early intervention.

  • Ensure annual review of the participant's physical condition, and encourages compliance with treatment for identified physical problems.

  • Provide on-call crisis intervention coverage as assigned.

  • Monitor medication, documenting participants compliance, medication efficacy, and side effects as reported. Supervises participants in self administration of medication as needed.

  • Registered nurse with a valid nursing license in the state of Delaware.


Job Qualifications:
  • Registered nurse with a valid nursing license in the state of Delaware.

  • Bachelor's degree in nursing preferred.

  • Skills and competence to establish supportive and trusting relationships with persons with severe and persistent mental illness and respect for participants' rights and personal preference in treatment are essential.

  • Knowledge of mental health and substance abuse systems and supports.

  • Ability to work independently and as part of a focused team.

  • Good communication skills both oral and written.

  • Strong organizational and analytical skills.

  • Possess a valid driver's license with an acceptable driving record.

Scope:

  • Frequent talking and listening.
  • Requires the ability to lift, carry, hold, kneel, reach, stack, stoop and bend, which may include direct intervention with participants.
  • Local travel required, including transporting program participants.
  • May be required to report to work during emergencies, including inclement weather.
  • May require crisis intervention in emergency situations.
  • Scheduling flexibility and on-call responsibilities.
