Community Health Worker

The Community Health Worker at MGH Revere provides outreach to patients in the Pediatrics and Med-Peds departments. The Community Health Worker helps pediatric patients and their family's access and navigate some complex systems such as special education, mental health services, food resources, transitional assistance and housing. Other responsibilities include advocacy for patients and their families as well as case management. The Community Health Worker is also available to Pediatricians for consultation regarding other psychosocial issues including school failure, chronic missed appointments and DCF cases. The Community Health Worker meets with families individually to identify their assets and needs in order to facilitate the process of strengthening families.

Principal Duties & Responsibilities

  • Develop a relationship with pediatric patients and their families to support them in solving problems related to concrete needs such as transportation issues, school difficulties and DCF involvement.
  • Work closely with medical staff to ensure these needs are being addressed.
  • Educate families on available resources such as Mass Health, housing, SNAP, WIC, DTA, GED classes, literacy programs and after-schools programs.
  • Assist with new referrals to appropriate resources and improve linkages with existing services.
  • Identify gaps in services as well as assets and resources for families in this community to facilitate the process of strengthening families.
  • Work with families to identify barriers to care.
  • Provide a connection among providers, school staff, after school programs, as well as offer additional advocacy for families.
  • Guide families through the application process for Head Start or the public preschool program.
  • Assist families in initiating the Special Education evaluation process, organizing re-evaluations or team meetings for patients who already have an Individualized Education Plan (IEP) and/or discussing school progress with the child's pediatrician, parents and teachers. On occasion will attend IEP meetings as needed.
  • Make mental health referrals from pediatricians to outside facilities that are specific to the needs of the patient including In-Home Therapy, outpatient therapy and substance use support programs.
  • Assist with referrals for home-based Autism supports such as Applied Behavioral Analysis (ABA) programs and In-home Behavioral Support (IHBS) services and coordinate support services for pediatric patients in the practice with a diagnosis of autism.
  • Distribute donations to families for baby items, clothing, books, backpacks and toys through various partnerships with organizations such as Cradles to Crayons and oversee the distribution of donations to pediatric patients during the health center's annual gift drive.
  • Identify, develop and maintain relationships with organizations throughout the community. This will include, but may not be limited to, Revere Public Schools, Chelsea-Revere Family Network, North Suffolk Mental Health, recreational activities, and additional community-based resources.
  • Perform other duties as assigned by the Program Manager/ Unit Chief.
  • Enter notes of intervention into the appropriate electronic health record.

Qualifications
  • Qualifications
  • Bachelor or four-year degree in psychology, social work or a related field is recommended
  • Bilingual skills preferred (Spanish or Arabic)
